544 Vasculitic Subglottic Stenosis: A Question of Immunosuppression?

Abstract Introduction Subglottic stenosis (SGS) is the commonest manifestation of tracheobronchial disease in granulomatosis with polyangiitis (GPA) and carries a high degree morbidity. Management SGS-GPA double-edged sword. Delayed treatment may cause respiratory compromise infectious complications. However, aggressive surgical management initiate systemic inflammatory response, reactivating vasculitic cascade potentially lead to long-term complications including renal vasculitis consequential end-stage failure. There currently no internationally agreed strategy for this disease. Method This retrospective review was undertaken analyse our unique combination dilatations immunosuppressive-focused adjuvant between years 2011-2020. Results Sixteen one hundred nine GPA patients (14.7%) had SGS were included analysis. Whilst three (18.8%) improved solely on medical treatment, thirteen (81.3%) required combined surgery immunosuppression (consisting cyclophosphamide or Rituximab regimens). Thirty-nine two tracheostomies performed over mean 53-month follow-up period, calculated procedure rate every 24.8 months (2.7 - 89 months). Conclusions Our current affords lower at compared other published studies 14.9 months.
